Job summary

Qualifications

• Licensed RN in the State of Michigan.
• Experience in acute care, home care, or hospice preferred.
• Strong health assessment, organizational, and interpersonal skills.
• Able to meet Home Health and Medicare standards with background check.
• Valid Michigan driver’s license and insured vehicle for patient visits.

Responsibilities

• Manage an assigned caseload from admission through discharge using assessment, planning, coordination, and evaluation.
• Complete comprehensive assessments (including OASIS when required) and develop plans of care.
• Provide skilled nursing services across preventative, rehabilitative, palliative, and end-of-life needs.
• Educate and counsel patients and families on disease processes, symptom management, and self-care; coordinate with interdisciplinary team.
• Supervise LPNs and paraprofessionals; document thoroughly in EMR.
• Communicate changes in patient condition to physicians and team.
