Job summary

Munson Healthcare in northern Michigan is seeking a dedicated Registered Nurse to manage a patient caseload from admission to discharge, delivering skilled nursing services across preventive, rehabilitative, palliative, and end‑of‑life needs.

You will educate patients and families, coordinate with Home Health Aides, Therapies, Social Work, and Chaplaincy, supervise LPNs and paraprofessionals, and ensure thorough documentation in the electronic medical record. Michigan license is required.

Qualifications

• Licensed RN in Michigan.
• Experience in acute care, home care, or hospice preferred.
• Proven health assessment, organizational, and interpersonal skills with the ability to make independent judgments directing patient care.
• Able to meet Home Health and Medicare standards through criminal background check and fingerprinting.

• Completes comprehensive assessments and develops individualized plans of care; provides skilled nursing services across preventive, rehabilitative, palliative, and end‑of‑life needs.

• Supervises LPNs and paraprofessionals, communicates changes to physicians and team, secures equipment and supplies, and maintains thorough documentation in the EMR.

A Day in the Life

• Responsibilities include completing comprehensive assessments (including OASIS when required), developing and updating individualized plans of care, and providing skilled nursing services across preventative, rehabilitative, palliative, and end-of-life needs.

• Educates and counsels patients and families on disease processes, symptom management, and self-care, and coordinates care with interdisciplinary team members such as Home Health Aides, Therapies, Social Work, Chaplaincy, and Bereavement Services.

• This position also supervises LPNs and paraprofessionals, communicates changes in patient condition to physicians and team members, secures necessary equipment and supplies, and maintains thorough, timely documentation within the electronic medical record.

Qualifications

What’s Required

• Licensed RN in the State of Michigan

• Experience in acute care, home care, or hospice preferred

• Proven health assessment, organizational, and interpersonal skills with the ability to make independent judgments directing patient care.

• Able to meet Home Health and Medicare standards through criminal background check and fingerprinting.

• Possess current Michigan motor vehicle license, ability to drive a car and an insured vehicle capable of transport to patient homes or other offices in various weather conditions.
