Qualifications

• Experience in acute care, home care, or hospice preferred.
• Ability to make independent judgments directing patient care.
• Must meet Home Health and Medicare standards.

Responsibilities

• Manages an assigned caseload from admission through discharge.
• Completes comprehensive assessments and develops individualized plans of care.
• Educates and counsels patients and families on disease processes.
