Overview

On Call Registered Nurse (7 days on, 7 days off) At Ancora Home Health & Hospice we are expanding our Home Health & Hospice Nursing Teams and are seeking a dynamic and experienced On Call Registered Nurse who shares our desire to make healthcare better for everyone. COMPENSATION: $40,000/base salary + pay per visit. STATUS: On Call. SCHEDULE: 7 days on and 7 days off (Monday-Thursday 5pm-8am and then Friday 5pm-Monday 8am). LOCATION: Anchorage & Wasilla AK.

Job Summary

As a RN Case Manager at Ancora, you will play a crucial role in delivering exceptional home health and end-of-life care to our patients. Your compassionate approach will ensure comprehensive physical, social, and psychological assessments and your friendly and supportive demeanor will actively engage patients, families, and the rest of our Interdisciplinary Group in developing comprehensive care strategies and plans. You will coordinate patient care delivery, manage resources, and maintain meticulous patient records, collaborating with team members, patients, and families to create a holistic care experience.

Responsibilities

• On-call responsibilities during scheduled periods to address all patient needs, including medically necessary scheduled visits.
• PRN (as-needed) visits and new patient admissions.
• Responding to and managing death calls.
• Coordinate care delivery, manage resources, and maintain meticulous patient records.
• Collaborate with the interdisciplinary team to develop and implement comprehensive care plans.

Schedule & Commitments

Schedule: 7 days on, 7 days off rotation. Minimum commitment: must work a minimum of 15 days per month, including 2 weekends.

Qualifications

• Current Registered Nurse (RN) license in the state of practice.
• Home Health / Hospice experience as a Registered Nurse preferred.
• Ability to utilize full range of movement to transfer patients.
• Effective verbal & written communication and time management skills.
• Valid Driver's License (clean record) and reliable transportation.
• Proof of current auto insurance & registration.
• Current AZ Fingerprint Clearance Card (we can help).
• Current CPR Certification (we can help) & Negative TB Test (we can test).

Compensation & Benefits

• Salary: $44 - $47 per hour (note: the description also lists $40,000 base + pay per visit; final compensation may be adjusted based on market and role).
