Job summary

Sonder Healthcare seeks an experienced Home Health Registered Nurse (RN) to provide skilled nursing care in patients' homes across Sacramento County. This per diem role offers schedule flexibility and autonomy in the field.

Your responsibilities include assessments, care planning, medication administration, patient and caregiver education, and coordination with physicians. Valid CA RN license and BLS are required.

Qualifications

• Active California RN license (required)
• Minimum of three (3) years nursing experience
• Home health experience required
• Current BLS certification (American Heart Association)
• Valid driver’s license and reliable, insured transportation
• Strong clinical, documentation, and communication skills

Responsibilities
