The Registered Nurse (RN) in Home Health provides and directs nursing care to patients in their homes as prescribed by the physician and in compliance with laws, regulations, and agency policies. The RN coordinates the total plan of care with other healthcare professionals and ensures continuity of care by planning and exchanging information with physicians, agency personnel, patients, families, and community resources, all within a Point of Care setting.

Responsibilities include:

• Providing clinical services within the scope of practice, as defined by state laws, in accordance with the plan of care, and in coordination with other team members.
• Conducting initial and/or comprehensive nursing evaluations, determining primary focus of care, developing the plan of care within state guidelines, and submitting documentation per policy.
• Regularly evaluating and re-evaluating patient needs, at least every 60 days or as condition changes.
• Performing comprehensive assessments and developing appropriate plans of care in collaboration with physicians.
• Ensuring patients meet home health eligibility and medical necessity guidelines.
• Initiating, developing, implementing, and revising the plan of care with healthcare team collaboration.

License Requirements

• Current RN licensure in the practice state
• Current CPR certification
• Valid driver’s license, vehicle insurance, and access to transportation
