MEO Staffing, Inc. in Philadelphia, PA is seeking a Nurse Supervisor to lead comprehensive patient assessments, develop and evaluate individualized care plans with interdisciplinary teams, and oversee skilled nursing services in home health settings.

The role includes supervising LPNs, home health aides, and support staff; coordinating urgent responses; monitoring patient changes; and ensuring precise, compliant clinical documentation in line with agency standards and regulations.

Responsabilidades

• Patient assessments: Conduct comprehensive physical, emotional, and psychosocial assessments.
• Care planning: Develop and evaluate individualized care plans alongside interdisciplinary teams.
• Skilled nursing: Provide direct care, including wound management, IV therapy, and symptom/pain control.
• Family support: Deliver tailored education and emotional support to patients and caregivers.
• Team supervision: Oversee LPNs, home health aides, and support staff.
• Urgent response: Manage patient admissions, urgent clinical needs, and on-call rotations.
• Condition monitoring: Track patient changes and dynamically update care plans.
• Compliance & records: Maintain precise clinical documentation in alignment with agency standards and regulations.
