Responsibilities

Conduct comprehensive patient assessments
Develop and evaluate individualized care plans with interdisciplinary teams
Provide direct skilled nursing care including wound management and IV therapy
Deliver education and emotional support to patients and caregivers
Oversee LPNs, home health aides, and support staff
Manage patient admissions, urgent clinical needs, and on-call rotations
Monitor patient changes and update care plans accordingly
