Job summary

Ascend Health is hiring a Registered Nurse for Bergen and Essex Counties to assess, plan, and direct home care services. The RN will educate patients and families, coordinate care, and ensure compliance with agency protocols while traveling across assigned counties.

The role requires an RN license, experience in home care and OASIS, and reliable transportation. A $3,500 sign-on bonus is offered along with a salary range of 90–110K annually.

Qualifications

• Must have a State Certified RN license.
• Experience in Certified Home Care and OASIS.
• Must be willing to travel.
• Must have a valid driver’s license and reliable transportation.

Responsibilities

• Completes an initial assessment of patient and family to determine home care needs.
• Regularly re-evaluates patient nursing needs.
• Initiates the plan of care and makes necessary revisions as patient status changes.
• Uses health assessment data to determine nursing diagnosis.
• Develops a care plan with goals based on nursing diagnosis and includes patient/family in planning.
• Supervise LPN and HHA documentation of care and services rendered.
• Provides health care instructions to patient as appropriate.
• Identifies discharge planning needs and implements prior to discharge.
• Report incidents timely per agency policy.
• Communicates with physician regarding patient needs and changes in condition.
• Participates in case conferences to coordinate plan of care.
• Participates in on-call duties as needed.

The Registered Nurse plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. Essential job duties can include, but are not limited to:

1. Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
2. Regularly re-evaluates patient nursing needs.
3. Initiates the plan of care and makes necessary revisions as patient status and needs change.
4. Uses health assessment data to determine nursing diagnosis.
5. Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
6. Supervise LPN and HHA documentation of care and services rendered according to agency policies and procedures.
7. Provides health care instructions to the patient as appropriate per assessment and plan of care.
8. Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
9. Report incidents timely and to appropriate persons per agency policy.
10. Communicates with the physician regarding the patient's needs and reports any changes in the patient's condition; obtains/receives physician's orders as required.
11. Participates in case conferences with other disciplines to coordinate plan of care.
12. Participates in on -call duties as needed.

Education:

Associate’s Degree in Nursing, Bachelor’s preferred
