Job summary

Ascend Health is seeking a Registered Nurse for PRN home-care assignments in Bergen and Essex Counties, NJ. The RN will plan, organize and direct home care services, drawing on community resources to meet patient needs in their homes and communities.

Responsibilities include comprehensive assessments, care plan development, supervision of LPN/HHA, patient education, discharge planning, and collaboration with physicians. Travel required, valid driver’s license essential.

Qualifications

• Must hold state RN license.
• Experience in Certified Home Care and OASIS.
• Willing to travel and have reliable transportation.
• Valid driver's license required.

Responsibilities

• Complete initial patient and family assessment to determine home care needs.
• Regularly re-evaluate nursing needs and revise the plan of care.
• Develop and update care plans with goals based on nursing diagnosis.
• Supervise LPN and HHA documentation per agency policies.
• Provide health care instructions to patients and families.
• Identify discharge planning needs and ensure planning prior to discharge.
• Report incidents per agency policy; communicate with physician.
• Participate in case conferences and on-call duties.

The Registered Nurse plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. Essential job duties can include, but are not limited to:

1. Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
2. Regularly re-evaluates patient nursing needs.
3. Initiates the plan of care and makes necessary revisions as patient status and needs change.
4. Uses health assessment data to determine nursing diagnosis.
5. Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
6. Supervise LPN and HHA documentation of care and services rendered according to agency policies and procedures.
7. Provides health care instructions to the patient as appropriate per assessment and plan of care.
8. Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
9. Report incidents timely and to appropriate persons per agency policy.
10. Communicates with the physician regarding the patient's needs and reports any changes in the patient's condition; obtains/receives physician's orders as required.
11. Participates in case conferences with other disciplines to coordinate plan of care.
12. Participates in on -call duties as needed.

Education:

Associate’s Degree in Nursing, Bachelor’s preferred

Professional Experience:

• Must have a State Certified RN license
• Experience in Certified Home Care and OASIS
• Must be willing to travel
