Join Pinnacle Home Care – Now Hiring Full-Time & PRN Registered Nurses in Manatee County, FL!

Key Responsibilities:

As a Pinnacle RN, you will:

• Deliver high-quality, compassionate nursing care in accordance with individualized care plans.
• Perform health and psychosocial assessments to support patient recovery.
• Collaborate with physicians, discharge planners, and caregivers to educate and coordinate care.
• Monitor patient conditions, promote wellness, and provide direct nursing interventions.
• Maintain clear, accurate documentation, including OASIS assessments.
• Act as a liaison between patients and community health resources.

What You’ll Need:

• Active Florida RN License.
• Minimum 1 year of nursing experience in home health or clinical care.
• Oasis Documentation experience.
• Strong communication, time management, and clinical documentation skills.
• Valid driver’s license and reliable transportation.

Physical Requirements:

• Ability to perform home visits, including in non-accessible environments.
• Capacity for physical activity: walking, standing, lifting, stooping, and occasional patient support.
