Facility: Providence Home Health - LA County North

Job Type: Contract

Start Date: ASAP

Shift: 5x8-Hour Day Shifts (08:00 AM - 4:30 PM)

Guaranteed Hours: Facility may cancel up to one shift per pay period (bi-weekly)

Job Description:

We are seeking an experienced Home Health Registered Nurse (RN) to join our team serving patients in the Santa Clarita area and nearby locations. This is a day shift, Monday through Friday position with occasional weekend requirements.

Key Responsibilities:

• Provide 1:1 care in a skilled home health setting
• Visit up to 6 patients per day (30 productivity points per day standard)
• Accurately document all care and communication using EPIC/OASIS
• Drive between patient homes - own reliable transportation required
• Ensure top-tier care while adhering to safety and compliance standards
  
  

Requirements:

• Active RN License (Pending license accepted)
• 1+ year of skilled home health experience (OASIS experience required - please highlight in application)
• BLS Certification required
• Must be able to work 1 weekend shift during the contract
• Must be comfortable driving throughout assigned region (occasional 50+ mile radius)
• First-time travelers not accepted
