Our Registered Nurses, Therapists, and Social Worker coordinate care in Lake & McHenry County in Illinois and Kenosha, Racine & Walworth County in Wisconsin. We work with hospitals,skilled nursing providers and primary physicians to continuously assess medical conditions; ensure medication compliance by educating patients and caregivers on the purpose, frequency, proper administration, and possible side effects of medications; all while maintaining a level of care, compassion and consistency that dramatically improves the quality of life of our patients. Job Description

Position requirements

• Performs assessments and develops individualized patient care plans and manages multi-disciplinary care as applicable.
• Provides skilled nursing care in a patient’s home as ordered by the attending physician in conjunction with the plan of care.
• Supervises LPN’s and Home Health Aide’s in a patient’s home.
• Complies with agency policies and procedures.
• Promotes and maintains an agencyenvironment that is in compliance with federal, state, and local regulations.
• Submits clear and appropriate documentation in a timely manner and documents patient education.
• Communicates effectively to physicians, supervisors, and coworkers in a timely manner.
• Participates in case conferences to promote coordination of care.
• Promotes continuity of care with appropriate admissions, transfers and discharges.
• The RN will attend in-service training and attend mandatory agency meetings.
• Reports to appointments as scheduled.
• Maintains a professional appearance
• Perform all aspects of patient care.

Qualifications

• Registered Nurse with current license in the state of Illinois, required.
• Registered Nurse with current license in the state of Wisconsin, preferred.
• One year of experience as an RN in an acute care setting, preferably two years of experience.
• Must have at least one year of Home Health Experience.
