Job summary

Ashton North LLC seeks an experienced Home Health RN Case Manager to support the Dallas/Fort Worth area field-based caseload. The role requires strong assessment skills, timely documentation, and independent management of patient care in the home setting.

The ideal candidate holds an active Texas RN license with recent home health case management experience and is capable of coordinating care across physicians, families, and the interdisciplinary team. Local travel is required.

Qualifications

• Active Texas RN license in good standing.
• Recent Home Health RN Case Management experience.
• Strong understanding of Medicare home health, OASIS, plan-of-care management, and field-based nursing.
• Ability to travel locally throughout Fort Worth / Tarrant County.

Responsibilities

• Manage an assigned home health patient caseload in the field.
• Perform skilled nursing visits, assessments, evaluations, recertifications, resumptions of care, and discharges.
• Develop, update, and coordinate individualized plans of care with physicians, patients, families, and the care team.
• Complete accurate, timely clinical documentation, including OASIS and visit notes.
• Monitor patient progress and communicate updates to the team.
• Educate patients and caregivers on disease management, medications, safety, and plan-of-care goals.
• Coordinate care with therapy, social work, aides, physicians, and leadership.
• Maintain compliance with home health regulatory, documentation, and quality standards.

Home Health RN Case Manager – Dallas, TX

Territory: Dallas area south of Downtown Dallas, including Cedar Hill, DeSoto, Red Oak, and Duncanville

We are seeking an experienced Home Health RN Case Manager for a field-based role serving Fort Worth, TX and the surrounding Tarrant County area. This position is best suited for a clinically strong Registered Nurse with recent home health case management experience, strong assessment skills, solid documentation habits, and the ability to manage an assigned patient caseload independently in the field.

Key Responsibilities

• Manage an assigned home health patient caseload in the field.
• Perform skilled nursing visits, assessments, evaluations, recertifications, resumptions of care, and discharges.

Required Qualifications

• Active Texas RN license in good standing.
• Recent Home Health RN Case Management experience required.
• Strong understanding of Medicare home health, OASIS, plan-of-care management, and field-based nursing.
• Ability to work independently while maintaining strong communication with the clinical team.
• Strong clinical judgment, organization, documentation, and time-management skills.
• Valid driver’s license, reliable transportation, and ability to travel locally throughout Fort Worth / Tarrant County.
• EMR experience preferred.
