Job summary

Pathways Healthcare is seeking dedicated registered nurses for full-time, day-shift positions in Fall River, MA. You will provide in-home nursing care, write and update plans of care, assess patients, and coordinate with physicians to ensure optimal outcomes.

The role emphasizes family involvement, safe patient environments, and ongoing professional development, with on-call duties and opportunities for leadership growth.

Qualifications

• 2+ years of nursing experience (geriatrics desirable).
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records (EMR).
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a vehicle.

Responsibilities

• Assess and evaluate patient status; write and initiate plan of care.
• Regularly re-evaluate patient and family needs; revise plan as necessary.
• Develop care plans based on nursing diagnosis and family involvement.
• Administer medications and treatments per physician’s plan of care.
• Complete and submit accurate clinical notes; track pain and symptoms.
• Communicate patient needs to physician; obtain orders as required.
• Maintain safe patient environment; attend interdisciplinary meetings.
• Participate in on-call duties and ensure equipment needs are met.
• Supervise ancillary staff; delegate tasks as needed.

Position Details:

• Main Office location: Fall Rive, MA
• Service Area: Fall River, Freetown, Middleboro, Wareham, New Bedford, Lakeville, Somerset, Fair Haven and Rochester areas.
• Job Type: Full Time
• Schedule: Monday to Friday

Responsibilities:

Patient Care:

• Assesses and evaluates patient’s status by:
• Writing and initiating plan of care.
• Regularly re-evaluating patient and family/caregiver needs.
• Participating in revising the plan of care as necessary.
• Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
• Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate.
• Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
• Provides and maintains a safe environment for the patient.
• Attends interdisciplinary group meetings.

Additional Duties:

• Participates in on-call duties.
• Ensures that arrangements for equipment and other necessary items and services are available.
• Supervises ancillary personnel and delegates responsibilities when required.
• Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
• Fulfills the obligation of requested and/or accepted case assignments.
• Actively participates in quality assessment performance improvement teams and activities.

Qualifications:

• 2+ years' nursing experience, preferably in geriatrics is desirable.
• New nurses, feel free to apply! We have a great training program.
• Degree in Nursing from an accredited institution.
