Job summary

Puget Sound Home Health & Hospice in Buckley, WA is seeking multiple Home Health Registered Nurses (RN) to join our compassionate team, delivering skilled care in patients’ homes. Qualifications

• Current Registered Nurse (RN) license.
• Current AHA BLS certification.
• Home Health and/or Hospice experience preferred.
• Valid driver’s license, insured vehicle, and reliable transportation.
• Ability to work independently while collaborating with a clinical team.

Responsibilities

• Assess patients in their homes and identify their individual care needs.
• Develop and manage personalized plans of care in collaboration with providers.
• Provide skilled nursing care, education, and support to patients and families.
• Coordinate care with therapists, physicians, and other members of the interdisciplinary team.
• Monitor patient progress and adjust care as needs change.
• Complete accurate, timely clinical documentation.
• Participate in quality initiatives that help us continually improve patient care.

We’re growing and expanding our service areas, and we’re looking for multiple RNs to join our Home Health team , with a current focus on Buckley, Puyallup, and Graham.

Compensation

What You’ll Do
