Perform patient assessments (initial + ongoing)
Develop and update care plans
Administer meds and manage symptoms
Educate/support patients and families
Attend/participate in IDT meetings
Make home visits as needed
Document in EMR accurately and timely
Communicate with physicians and team
Triage urgent calls/on-call rotation
Guide on hospice philosophy/DNR/advance directives
Provide end-of-life emotional support
Mentor new staff
Ensure compliance with regulations
Coordinate with outside services (DME, pharmacy, etc.)
Registered Nurse (RN) licensure
**Prior experience working in a hospice agency is strongly preferred**
Ability to work independently and demonstrate critical thinking
Current BLS certification from AHA
Be a licensed driver with an insured automobile in good working order
