Job summary

Puget Sound Home Health and Hospice in King County, Washington (Federal Way area) is seeking an experienced and caring full-time Hospice Registered Nurse Case Manager (RN).

Responsibilities include patient assessments, care planning, symptom management, education for patients and families, and IDT participation, with competitive benefits and a salary range of $115,000 to $125,000 per year.

Qualifications

• RN licensure in good standing
• Hospice experience preferred
• Ability to work independently and demonstrate critical thinking
• Current BLS certification
• Must have a valid driver’s license and insured vehicle

Responsibilities

• Perform patient assessments (initial + ongoing)
• Develop and update care plans
• Administer meds and manage symptoms
• Educate and support patients and families
• Attend/participate in IDT meetings
• Make home visits as needed
• Document in EMR accurately and timely
• Communicate with physicians and team
• Triage urgent calls/on-call rotation
• Guide hospice philosophy and advance directives
• Provide end-of-life emotional support
• Mentor new staff
• Ensure compliance with regulations
• Coordinate with outside services (DME, pharmacy, etc.)

Essential Duties

Requirements and Qualifications

• Registered Nurse (RN) licensure

• **Prior experience working in a hospice agency is strongly preferred**

• Ability to work independently and demonstrate critical thinking

• Current BLS certification from AHA

• Be a licensed driver with an insured automobile in good working order
