Ascendconciergehomecare is hiring a Registered Nurse for Bergen and Essex Counties in New Jersey. Salary ranges from $90,000 to $110,000 per year with a $3,500 sign-on bonus.

The RN will plan, organize, and direct home care services, leveraging community resources to meet patient needs in home settings. Requirements include Associate's Degree in Nursing, State Certified RN license, experience in Certified Home Care and OASIS, willingness to travel, and a valid driver’s license with reliable

Responsibilities

• Completes initial assessment of patient and family to determine home care needs.
• Re-evaluates patient nursing needs regularly.
• Initiates and revises the plan of care as patient status changes.
• Coordinates with physicians and other disciplines to implement the plan of care.
• Provides health care instructions to patients and families.
• Participates in discharge planning and case conferences.
• Participates in on-call duties as needed.

Now Hiring – Registered Nurse – Bergen and Essex Counties

The Registered Nurse plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. Essential job duties can include, but are not limited to:

1. Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
2. Regularly re-evaluates patient nursing needs.
3. Initiates the plan of care and makes necessary revisions as patient status and needs change.
4. Uses health assessment data to determine nursing diagnosis.
5. Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
6. Supervise LPN and HHA documentation of care and services rendered according to agency policies and procedures.
7. Provides health care instructions to the patient as appropriate per assessment and plan of care.
8. Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
9. Report incidents timely and to appropriate persons per agency policy.
10. Communicates with the physician regarding the patient's needs and reports any changes in the patient's condition; obtains/receives physician's orders as required.
11. Participates in case conferences with other disciplines to coordinate plan of care.
12. Participates in on -call duties as needed.

Education:

Associate's Degree in Nursing, Bachelor's preferred
