Job summary

Bergen Care Home Health is seeking a Registered Nurse for PRN shifts to support home-based patient care in New Jersey. The RN will plan, coordinate, and deliver nursing services in patients' homes, emphasizing community health education and compassionate care.

The role requires an Associate’s Degree in Nursing (Bachelor’s preferred), a State Certified RN license, and willingness to travel. Travel and on-call duties are part of the PRN assignments, with supervision of LPNs and HHAs as applicable.

Qualifications

• Must hold a State Certified RN license.
• Experience in Certified Home Care and OASIS is preferred.
• Willingness to travel to patient homes is required.
• Valid driver’s license and reliable transportation are required.

Responsibilities

• Complete initial assessment of patient and family to determine home care needs.
• Regularly re-evaluate patient nursing needs.
• Initiate and revise the plan of care as patient status changes.
• Use health data to determine nursing diagnosis and care plans.
• Supervise LPN and HHA documentation of care per policies.
• Provide health care instructions to patients per plan of care.
• Identify discharge planning needs and implement prior to discharge.
• Report incidents per agency policy.
• Communicate with physicians regarding patient needs and changes.
• Participate in case conferences to coordinate plan of care.
• Participate in on-call duties as needed.

The Registered Nurse plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. Essential job duties can include, but are not limited to:

1. Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
2. Regularly re-evaluates patient nursing needs.
3. Initiates the plan of care and makes necessary revisions as patient status and needs change.
4. Uses health assessment data to determine nursing diagnosis.
5. Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
6. Supervise LPN and HHA documentation of care and services rendered according to agency policies and procedures.
7. Provides health care instructions to the patient as appropriate per assessment and plan of care.
8. Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
9. Report incidents timely and to appropriate persons per agency policy.
10. Communicates with the physician regarding the patient's needs and reports any changes in the patient's condition; obtains/receives physician's orders as required.
11. Participates in case conferences with other disciplines to coordinate plan of care.
12. Participates in on -call duties as needed.

Education:

Associate’s Degree in Nursing, Bachelor’s preferred
