Responsibilities

• Provide skilled nursing care in patients' homes per physician orders and plans of care.
• Complete initial and ongoing nursing assessments and update plans of care.
• Monitor patient conditions and coordinate needed services with physicians.
• Educate patients and families about care and health needs.
• Complete clinical documentation, OASIS and Medicare documentation.
• Coordinate care with PT, OT, SLP, social workers and others.
• Supervise LPNs and CNAs as assigned.
• Participate in patient care conferences and quality improvement activities.

Location: Bellevue & Medina, WA / King County

Schedule: Full-time, Monday-Friday; on-call as assigned

Pay and benefits: $50-$62.05/hr DOE + mileage reimbursement; medical, dental, vision & life insurance, 401(k) match, 16 days PTO + holidays, paid sick leave & more

Job Description:

• Provide skilled nursing care to patients in their homes according to physician orders and individualized plans of care.
• Complete initial and ongoing nursing assessments and develop/revise plans of care.
• Monitor patient conditions, communicate changes to physicians and coordinate needed services.
• Educate patients and families regarding care, treatment and health needs.
• Complete clinical documentation, OASIS assessments and required Medicare documentation.
• Coordinate care with PT, OT, SLP, social workers and other healthcare professionals.
• Supervise and provide guidance to LPNs and CNAs as assigned.
• Participate in patient care conferences and quality improvement activities.

Requirements:

• Current Washington State RN license.
• 1+ year of nursing experience preferred.
• Home health experience preferred.
• Ability to travel throughout assigned areas of King County.
• Valid driver's license and reliable, insured vehicle.
