Job summary

Favorite Healthcare Staffing Inc. is seeking an experienced Registered Nurse (RN) for a full-time Home Health position in Bellevue, WA. The role provides skilled nursing care in patients' homes across King County, with scheduling Monday–Friday and on-call as assigned.

Responsibilities include assessments, plans of care, patient education, documentation, and coordination with other professionals. Must hold WA RN license, travel ability, and reliable vehicle; mileage reimbursed and benefits

Qualifications

Current Washington State RN license.
1+ year of nursing experience preferred.
Home health experience preferred.
Valid driver's license and reliable vehicle.
Ability to travel throughout King County.

Responsibilities

Provide skilled nursing care to patients in their homes according to physician orders and individualized plans of care.
Complete initial and ongoing nursing assessments and develop/revise plans of care.
Monitor patient conditions, communicate changes to physicians and coordinate needed services.
Educate patients and families regarding care, treatment and health needs.
Complete clinical documentation, OASIS assessments and required Medicare documentation.
Coordinate care with PT, OT, SLP, social workers and other healthcare professionals.
Supervise and provide guidance to LPNs and CNAs as assigned.
Participate in patient care conferences and quality improvement activities.
