Qualifications

• Active NJ Registered Nurse (RN) License is required.
• At least 6 months of RN experience or 1 year of LPN experience preferred.

Primary Responsibilities

1. Conduct comprehensive nursing assessments and implement individualized care plans for patients in their homes.
2. Administer treatments and procedures per organizational standards and physician orders.
3. Educate patients and their families on care plans, treatments, and goals.
4. Collaborate with interdisciplinary teams to ensure effective care coordination.
5. Maintain accurate and timely documentation of assessments, interventions, and outcomes.
6. Supervise home health aides and complete regulatory supervisory visits.
7. Ensure compliance with payer guidelines and authorization requirements.
8. Act as a liaison between physicians, managers, and other care team members to optimize patient outcomes.
9. Participate in continuous improvement by incorporating updated standards into your practice.

Qualifications Required:

1. Associate's Degree in Nursing or Diploma Certificate in Nursing.
2. Active NJ Registered Nurse (RN) License.
3. AHA Basic Life Support (BLS) Certification.
4. At least six months of RN experience or one year of LPN experience, preferably in home care or hospice.
5. Proficiency in PC-based systems.

Preferred:

1. Bachelor’s Degree in Nursing (BSN).
