Responsibilities

The Registered Nurse in Home and Community Based Services provides and directs nursing and personal care to patients in their homes, as prescribed by the physician or authorized by the state/program, in compliance with applicable laws, regulations, and agency policies. The RN coordinates the care plan with other healthcare professionals and ensures continuity of care by sharing information with physicians, agency staff, patients, families, case managers, and community resources.

Specific Job Duties/Responsibilities

• Provides high-quality clinical services within the scope of practice and infection control standards, according to the care plan, and in coordination with the care team from admission through discharge.
• Completes clinical nursing assessments as per federal and state requirements and payer specifications.
• Verifies patient eligibility and medical necessity for services based on payer source and agency policy.
• Develops and updates individualized care and service plans in collaboration with community providers, following federal and state guidelines.
• Ensures the plan of care's frequency and duration meet patient needs and revises it with physician approval as necessary.

Education and Experience

Desired Experience: Minimum of one year as an RN preferred. License Requirements:

• Current RN licensure in the state of practice.
• Valid driver’s license, vehicle insurance, and access to reliable transportation.
• Current CPR certification.

State-Specific Requirements:

• LA: Minimum one year of clinical RN experience; may be waived for RNs with recent LPN clinical experience. RN licensure must be unrestricted.
• AR, AZ, CT, D.C., FL, KY, MA, MD, MO, NY, NC, NH, NJ, OH, PA, TN, TX, VA, WI: No additional state-specific requirements.
