Job summary

Mass General Brigham Home Care, Inc. is seeking a Registered Nurse to join the Lighthouse team, primarily serving Marblehead and Swampscott with coverage also in Beverly, Prides Crossing, and Salem.

You will oversee the full plan of care as a case manager, coordinating with an interdisciplinary team across visits, documentation, and care coordination. The role is full-time with a 40-hour workweek in a hybrid split between home visits and remote work, located at 152 Conant Street.

Qualifications

• Graduate of an approved School of Nursing.
• Current Massachusetts license as Registered Nurse.
• Minimum of one-year previous nursing experience in acute care setting; prior home care preferred.
• Travel required within MGB Healthcare at Home geographic area.
• Ability to work with various computer software; knowledge of hand-held patient care devices preferred.
• Strong interpersonal and customer service; ability to work with a diverse population.
• Ability to communicate effectively in writing, verbally and electronically.
• Ability to work as a member of an interdisciplinary team.
• Ability to work independently, be self-directed and adapt to unpredictable circumstances.
• Ability to organize and prioritize work.

Responsibilities

• Serve as case manager, overseeing the full plan of care while collaborating closely with LPNs, therapists, and social workers.
• Complete 5-6 visits per day, Monday through Friday, with time for documentation, chart review, and coordination.

While the majority of your time will be working in Marblehead and Swampscott, the entire team also supports Beverly, Prides Crossing, and Salem. You’ll typically complete 5-6 visits per day, Monday through Friday, with time for documentation, chart review, and coordination built into your schedule.

Mass General Brigham Home Care provides non-acute, medically necessary skilled care in the home to help “homebound” patients recover from illness or injury and manage exacerbations of chronic disease. Services include skilled nursing care (IV drug administration, injections, tube feeding, wound care, care plan education), physical therapy, occupational therapy, speech language pathology, medical social services, and as needed medical supplies.
