Qualifications

• Graduate of an accredited nursing school.
• Associate degree in nursing required; BSN preferred.
• Acute care experience highly preferred.
• Current Connecticut RN license.
• BLS certification by the American Heart Association.
• Excellent assessment and critical thinking skills.
• Must be able to commute to Bridgeport, CT.

Responsabilités

• Implements and monitors patient care plans and communicates patient condition.
• Serves as primary coordinator of all disciplines for well-coordinated care.
• Notes and carries out physician and nursing orders.
• Assesses and coordinates discharge planning needs with the healthcare team.
• Provides individualized care considering physical, psychosocial, cultural, spiritual, and age-related needs.
• Delegates care tasks to nursing staff to maximize performance.
• Participates in daily interdisciplinary patient rounds and provides input.
• Develops, implements, and revises plans of care as needed.
