Job summary

Home Health Care, Inc. in Kentucky is seeking a Registered Nurse to provide specialized care on a 21-bed unit, coordinating nursing care, patient education, and discharge planning for patients with GI disorders and chronic illnesses.

The RN will educate patients and families, lead a team, support quality improvement, and may drive a personal vehicle for work. TN sponsorship is available; state licensure and BLS are required.

Qualifications

• Graduate of an accredited nursing program.
• Proficiency in computer systems; EMR experience preferred.
• Valid RN license and BLS certification prior to start.
• Ability to work flexible hours and adapt to changing situations.

Responsibilities

• Deliver direct patient care with a holistic approach tailored to diverse patient needs.
• Educate patients and families to support health management and recovery.
• Participate in discharge planning and transition of care processes.
• Lead and support team activities, including mentorship and quality improvement.
