Job summary

Ambercare Homecare is seeking a Registered Nurse to join our growing team in Silver City, NM. This full-time role involves in-home visits from 8am to 5pm, Monday through Friday, with call rotation for after-hours coverage.

You will perform assessments, develop and implement plans of care, and coordinate with physicians and the interdisciplinary team to ensure optimal patient outcomes. A valid NM RN license and CPR are required, with home health experience preferred.

Qualifications

• Must be a graduate of an accredited nursing program and licensed in the state.
• At least 1 year of clinical experience preferred in home health, ICU, rehab, or med-surg.
• Must have CPR certification and reliable transportation.

Responsibilities

• Provide visits as assigned by the Clinical Services Manager.
• Assess eligibility, admissions, planning and evaluation as part of the nursing process.
• Coordinate with interdisciplinary team to ensure optimal patient care.

RN What Youll Do

• Provide visits as assigned by the Clinical Services Manager including the assessment determination of eligibility admissions processing planning implementation and evaluation phases of the nursing process
• Assignments will vary depending on licensure
• Obtain data and assess necessary information from patient physical psychological social and spiritual factors that may impact patient and familys needs and coordinate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician interdisciplinary team and other agencies for the purpose of coordinating optimal care
• Provide visits to assess the patients needs and update the plan of care
• Perform therapeutic preventative and rehabilitative nursing procedures as directed by the attending physician
• Perform clinical pronouncement of death and provide support to family as applicable to licensure
• Maintain accurate patient records charts progress notes reports and prepare care plans timely

Qualifications
