Job summary

All Heart Home Care is seeking a compassionate and experienced Registered Nurse (RN) to join our home health team in the Murfreesboro area. This full-time position delivers high-quality, patient-centered care in patients' homes while collaborating with a multidisciplinary health care team.

The role requires an active Tennessee RN license, CPR certification, and a valid driver’s license with reliable transportation.

Qualifications

• Current RN license in Tennessee.
• Current CPR certification.
• Valid driver's license, reliable transportation, and current auto insurance.
• Strong clinical assessment, communication, and organizational skills.
• Ability to work independently and manage a patient caseload effectively.
• Previous home health experience preferred but not required.

Responsibilities

• Perform in-home patient assessments and develop individualized plans of care.
• Provide skilled nursing services in accordance with physician orders and established care plans.
• Monitor patient conditions and communicate changes to physicians and the care team.
• Administer medications, treatments, and wound care as needed.
• Educate patients and caregivers on disease management, medications, and home care practices.
• Supervise and coordinate care provided by Licensed Practical Nurses and other clinical staff as appropriate.
• Ensure accurate and timely documentation in compliance with company policies and regulatory requirements.
• Coordinate care with physicians, therapists, and other healthcare professionals.
• Travel to patient homes within the assigned service area.
