Qualifications

• Currently Licensed Registered Nurse (RN) in state of practice required.
• Must hold and maintain a current CPR certification; CPR certification within 90 days of hire may be required.

Responsibilities

• Assure that effective quality nursing care is delivered using the nursing process.
• Identify needs and input for discharge planning and follow-up throughout length of stay.
• Develop individualized care plans in collaboration with residents and interdisciplinary care team.
• Implement plans cost-effectively with focus on resident-centered outcomes and appropriate resources for counseling and discharge planning.
• Collaborate with physicians and other health care professionals by sharing information relevant to plan of care.
• Deliver and maintain optimum resident care and comfort with current nursing practices.
