As a team member of our Senior Community Care (SCC) product, you will provide care to patients at home, including nursing homes and assisted living facilities for seniors. This role adds essential support to improve access to care.

Position Highlights
  • You will collaborate with interdisciplinary team members and support multiple providers. The RN works alongside nurse practitioners (NPs) and physician assistants (PAs).
  • This is a field-based position, primarily visiting members in nursing homes.
  • Standard Hours: Monday - Friday 8AM - 5PM; no nights, weekends, or holidays.
  • Location: Queens
Primary Responsibilities
  • Collaborate with care team members on changes in condition and care coordination.
  • Triaging member needs to appropriate care team members and levels of care.
  • Assist with transitions in outpatient and inpatient care.
  • Ensure complete and accurate documentation, medication reconciliation, nursing rounds, and risk assessments.

Required Qualifications
  • Associate degree or higher in Nursing (RN); BSN preferred.
  • Active, unrestricted RN license in the state(s) or the ability to obtain within 60 days.
  • Basic Life Support certification.
  • Reliable transportation to visit members in designated areas.
