Responsibilities

• Function as a caregiver for a specific patient population to achieve desired outcomes.
• Expand knowledge and skills, provide effective direct care as part of the interdisciplinary team to complex patients.
• Oversee care provided by other caregivers through delegation, validation, evaluation, and coordination.
• Coordinate patient care/unit activities among nursing peers, physicians, and support services.
• Collect comprehensive data pertinent to the patient's health or situation.
• Perform age and disease-specific assessments independently, focusing on physiologic, psychological, and cognitive status.
• Recognize clinical and diagnostic status changes.
• Identify variables in patient/family education needs based on age, disease, and cultural differences.
• Develop and coordinate a plan of care with strategies and interventions to attain outcomes.
• Communicate, coordinate, and implement the care plan.
• Provide ongoing communication with patient/family and healthcare team regarding patient's status and care plans.
• Implement age and disease-specific care promptly and effectively.
• Coordinate patient/family education and ensure understanding.
• Document assessments, care plans, interventions, and evaluations electronically in real-time.
• Administer medications utilizing advanced bar-scanning and documentation technologies.
• Participate in multidisciplinary patient discharges for safe transition and continuum of care.
• Evaluate progress toward outcomes, utilize critical thinking, and initiate interventions for changes in patient condition.

Qualifications

• Minimum of an associate degree in Nursing or a diploma in Nursing.
• BSN in progress or willing to acquire within three years of hire.
• At least six months of relevant nursing experience or active participation in Hackensack Meridian Health's Nurse Residency Program.
• Ability to learn quickly, adapt, demonstrate accountability, teamwork, ongoing education, and deliver high-quality patient care.

Preferred Qualifications

• National Certification in area of specialty when eligible.

Licenses and Certifications

• Valid NJ State Professional Registered Nurse License.
• BLS/CPR certification by American Heart Association within 60 days of hire.
