Department: Nursing

Position Summary

Seeking a Field Registered Nurse Supervisor to oversee Rockland County. This role is responsible for delivering high-quality home nursing services in alignment with the agency's philosophy, policies, and Standards of Community Health Nursing Practice. The position involves supervising field staff, implementing care plans, and ensuring optimal patient outcomes.

Essential Functions

  • Develop, implement, and oversee patient care plans while supervising field staff.
  • Observe, record, and report patient needs and concerns accurately.
  • Coordinate home care services, ensuring effective communication with patients and care providers.
  • Execute care plans established by the RN Case Manager and approved by the physician.
  • Maintain thorough, accurate, and compliant documentation.
  • Meet productivity standards as defined by the Director of Patient Services.
  • Monitor patient progress and collaborate with the RN Case Manager for care plan adjustments.
  • Administer and review medications, ensuring proper accountability.
  • Perform enteral feeding and IV infusions as prescribed.
  • Provide tracheostomy, catheter, and ostomy care per medical orders.
  • Assess the safety, cleanliness, and appropriateness of durable medical equipment and supplies.
  • Assist with essential household tasks that impact patient health, including meal planning, feeding assistance, light housekeeping, bed linen changes, and laundry.
  • Educate patients on self-care techniques.
  • Supervise home health aides as directed.
  • Maintain open communication with healthcare team members.
  • Adhere to company policies and procedures.
  • Complete required company and state-mandated forms and training in a timely manner.
  • Demonstrate professionalism in attitude, communication, and attire.
  • Maintain responsiveness to Sunshine Home Care inquiries and update the company on schedule changes.
  • Inform the office of any client updates, personal health changes, or schedule modifications.
  • For pediatric cases: Accompany patients to school for one-on-one care. If the patient is absent, report to the school for reassignment.
  • Qualifications:
  • Graduate of an accredited Registered Nursing (RN) program.
  • Active state licensure and registration as a Registered Professional Nurse.
  • A minimum of one year of home care and/or pediatric home care experience is preferred.

Skills And Abilities

  • Ability to read and interpret safety protocols, operating instructions, and procedural manuals.
  • Proficiency in medication calculations, weight measurements, volume, and distance conversions.
  • Strong organizational, communication, and interpersonal skills.
  • Capability to resolve problems involving multiple variables in standardized situations.
  • Knowledge of patient assessment and education techniques.
  • Understanding of disease processes, medication administration, and drug interactions.
  • Familiarity with therapeutic diets, infection control practices, and aseptic techniques.
  • Experience in catheter insertion and care.
  • Ability to communicate effectively with clients across diverse age groups and socioeconomic backgrounds.

Physical Requirements

  • Ability to travel via car or public transportation to various locations.
  • Prolonged periods of sitting at a desk and working on a computer.
  • Capacity to lift over 20 pounds as needed.
