Job summary

UChicago Medicine AdventHealth La Grange in Illinois is seeking a Registered Nurse for a part-time night shift. You will perform comprehensive clinical assessments, create patient-centered care plans, and coordinate care with a multidisciplinary team to optimize outcomes.

The ideal candidate has RN licensure, relevant ED/nursing experience, and the ability to manage multiple priorities while navigating Windows-based systems. Benefits begin Day One and the role supports professional development.

Qualifications

Associates of Nursing is required.
Bachelor's degree preferred.
RN license required.
ACLS/BLS certifications preferred/required as listed.

Responsibilities

Conduct comprehensive clinical assessments to evaluate patients' health.
Develop and implement individualized care plans based on assessments.
Coordinate multidisciplinary care among physicians, nurses, social workers and therapists.
Monitor progress and adjust care plans based on outcomes.
Communicate with patients, families and healthcare team to ensure informed care.

Job Description

Conduct comprehensive clinical assessments to evaluate patients' physical, mental, and emotional health.
Develop and implement individualized care plans based on assessment findings and patient needs.
Coordinate multidisciplinary care by facilitating collaboration among healthcare professionals, including physicians, nurses, social workers, and therapists.
Monitor patient progress, regularly review, and adjust care plans based on patient responses and outcomes.
Communicate effectively with patients, families, and the healthcare team to ensure everyone is informed and involved in the care process.

Knowledge, Skills, and Abilities

Skills required to work in the emergency department as defined by unit
Must be able to use a high level of abstract reasoning to assess patients, evaluate interventions, and revise the plan of care according to patient outcomes
Basic computer skills – must be able to navigate Microsoft Windows-based programs
Demonstrated proficiency in acute-care nursing, knowledge, and skills
Able to navigate and manage multiple priorities
