Responsibilities

• Regularly assesses patients' needs and provides skilled nursing services as prescribed by the physician authorized plan of care and accepted standards of nursing practice.
• Provides direct resident care, including medication administration, treatments, call lights, assisting with transfers, responding to alarms, etc.
• Evaluates effectiveness of treatment plan and collaborates with team to update the plan.
• Processes physician orders and performs physician-ordered care accurately and timely.
• Follows up regarding medication changes and order clarifications.
• Coordinates care provided by patient care technicians and maintains clear documentation.
