Job summary

Henry Ford Health is seeking a Registered Nurse (RN) in Community Care Services to provide comprehensive patient care, from admission through ongoing assessments, and to develop individualized care plans for patients on maintenance hemodialysis. You will advise families, coordinate care, and support care transitions within a robust community-based program.

Qualifications

  • Graduate of a state-approved school of nursing.
  • Current State of Michigan RN licensure.
  • At least 12 months of nursing care experience, including 3 months of experience providing nursing care to maintenance hemodialysis patients, prior to assuming charge responsibilities.

Responsibilities

  • Conduct admission and ongoing assessments of patients using physical and psychosocial assessment skills, diagnostic data, and medical evaluations.
  • Evaluate the effectiveness of care provided in relation to short and long-term patient goals.
  • Develop and implement individualized patient care plans, anticipating and addressing patient needs.
  • Execute nursing actions and medical orders to meet the complex needs of patients and their families, considering age-specific psychosocial and educational requirements.
  • Document care provided and patient responses accurately in the patient's record.
  • Assume Charge Nurse responsibilities, overseeing care provided by LPNs and non-licensed technical personnel.
