Job summary

Responsibilities include conducting patient assessments, developing training plans for PD or home hemodialysis, administering medications, reviewing labs, and coordinating follow-up with care teams.

Qualifications

• Graduate of accredited school of nursing.
• Current RN licensure in Michigan.
• 12 months RN experience plus 3 months in HD/PD (or dual modality) required.
• Charge nurse: 9 months nursing plus 3 months specialized dialysis experience.
• Travel to patient homes required.
• Basic computer knowledge and keyboarding skills required.

Responsibilities

• Conduct comprehensive patient assessments and identify care needs.
• Develop and implement PD or home hemodialysis training plans.
• Provide patient education on treatment procedures and medications.
• Assess and ensure patient competency in self-administered dialysis.
• Administer medications as ordered by nephrology team.
• Monitor labs and medications, report changes to physicians.
• Coordinate follow-up care with transplant centers and other providers.
• Demonstrate teaching skills to build patient confidence at home.
