Qualifications

• Active RN license required
• BLS, ACLS certifications required
• PRN/FT RN experience preferred

Responsibilities

• Assess and analyze health status of patients on the unit
• Perform comprehensive assessments and document per policy
• Develop and update individualized care plans with patients and families
• Coordinate with the health care team to implement plan of care
• Lead discharge planning and ensure timely follow-up
• Document interventions, patient responses, and outcomes
• Participate in Performance Improvement activities and Core Measures
