Qualifications

• Graduate of an accredited School of Nursing (ADN or BSN).
• Active RN license and BLS/CPR certification.
• Clinical experience in long-term, post-acute, rehabilitative, or geriatric nursing preferred.
• Proficient in English for documentation and patient safety.

Responsibilities

• Deliver high-quality nursing care under supervision to achieve positive outcomes.
• Supervise LPNs and CNAs and delegate appropriately.
• Collaborate with physicians and care teams to ensure safe discharge planning.
• Maintain accurate clinical documentation in PCC EHR.
• Adhere to HIPAA and safety guidelines.
