Intermountain Healthcare Inc. is seeking a Registered Nurse to assess, plan, implement, and evaluate patient care within a multidisciplinary team in Grand Junction, CO.

The role emphasizes evidence-based practice, documentation, and regulatory compliance. The RN will collaborate with physicians, families, and other care members to develop individualized plans, coordinate education and referrals, and ensure high-quality outcomes.

Qualifications

• Current RN license in state of practice with compact license transfer within 60 days.
• RNs with less than 12 months of experience must obtain BSN within four years.
• Basic Life Support (BLS) certification required.

Responsibilities

• Assess physical, social, and psychological status and gather information from the interdisciplinary team.
• Plan and develop an individualized care plan with patient, family involvement.
• Implement care by directing the interdisciplinary care team and coordinating resources.
• Evaluate patient response and revise the plan as needed.
• Maintain professionalism, documentation, and compliance with licensure and regulatory standards.
