• Bachelor of Science in Nursing (BSN) preferred; must be from an accredited institution; will be verified.

Responsibilities

• Assess physical, social, and psychological status and coordinate information from the interdisciplinary team.
• Develop individualized care plans with patient and family involvement.
• Coordinate care delivery, education, and referrals across the care continuum.
• Evaluate patient responses to interventions and adjust plans as needed.
