Qualifications

• Excellent telephonic and written communication.
• Strong research, analytical, and problem-solving abilities.
• Prefer 2+ years in managed care or case management.

Responsibilities

• Conducts clinical assessments with members and providers to develop a comprehensive plan of care.
• Outreaches to members, families and providers to engage in complex case management.
• Collaborates with interdisciplinary care team to promote high-quality, cost-effective outcomes.
• Identifies and coordinates resources and referrals within BCBSMN networks.
• Monitors and updates plans of care over time in accordance with protocols.
• Documents member data per regulatory standards and BCBSMN guidelines.
• Collaborates to address member health needs and cost drivers.
