Job summary

Bethesda Health Group in Missouri is seeking a registered nurse to accurately assess residents' health status and ongoing needs, coordinating care with an interdisciplinary team. You will develop dynamic care plans, document actions, and communicate updates to residents, families, and IDT members, including physicians, in a timely and professional manner.

The role requires critical thinking, strong clinical skills, and the ability to manage multiple priorities while maintaining patient safety.

Qualifications

• RN license in good standing and graduation from an accredited nursing program.
• CPR/First Aid certification required, or willing to obtain within six months.
• Long-term care experience preferred.
• Strong communication and time-management skills.
• Ability to lift heavy objects and assist with transfers.

Responsibilities

• Assess residents' health status on admission and during care.
• Develop and update individual care plans with an interdisciplinary team.
• Document assessments and care plans accurately in writing and electronically.
• Communicate plan to residents, families, and IDT members.
• Respond to changes in condition using SBAR framework.
• Assist with activities of daily living and provide safe resident care.
• Adhere to policies and participate in QAPI processes.
