• Provide patient education, triage support, and assist in care coordination

Key Responsibilities

• Triage patient calls and assess appropriate next steps
• Complete prior authorizations and manage related documentation
• Provide occasional direct patient care as needed
• Educate patients on care plans, medications, and health maintenance
• Support administrative tasks to enhance clinic workflow and efficiency

Required Qualifications

• Active RN license in the state of New Hampshire (must be valid at time of submission)
• Previous experience in a clinic or primary care setting preferred
• Strong communication and clinical assessment skills
• Local candidates only

Preferred Qualifications

• Experience with care coordination and patient triage in an ambulatory setting

Certifications

• BLS (Basic Life Support)
• Proof of COVID-19 vaccination and booster (declinations not accepted)
