Job summary

Nightingale Home Healthcare seeks an experienced Home Health Registered Nurse (RN) to provide skilled care in patients' homes across DuPage and Cook Counties, IL and surrounding areas. This full-time role offers independence, flexible scheduling, and meaningful one-on-one patient care.

The position requires Home Health experience, active RN license, and a willingness to manage an assigned caseload, with robust benefits and opportunities for advancement within a patient-focused organization.

Qualifications

• Active RN license required.
• Home health experience is required.
• OASIS and WellSky (Kinnser) experience preferred.
• Current CPR certification.
• TB test and physical within the last 12 months.
• Valid driver’s license and reliable transportation.

Responsibilities

• Conduct admissions and OASIS assessments.
• Develop and implement individualized plans of care.
• Provide skilled nursing services and coordinate with physicians.
• Supervise LPNs, Home Health Aides, and Personal Care Attendants as needed.
• Ensure HIPAA-compliant documentation.
• Coordinate referrals for additional services such as physical therapy and social services.
