Job summary

Nightingale Home Healthcare is seeking an experienced Home Health Registered Nurse (RN) to provide skilled nursing care and case management in the home. This role offers independence, flexible scheduling, and meaningful one-on-one patient care across DuPage & Cook Counties, IL.

The RN will manage an assigned caseload, perform assessments, and coordinate services with physicians and the care team. Home health experience is required and CPR certification is mandatory.

Qualifications

• Active Registered Nurse (RN) license.
• Home Health experience is required.
• OASIS and WellSky (Kinnser) experience preferred.
• Current CPR certification.
• Current TB test and physical within the last 12 months.
• Valid driver’s license with reliable transportation.

Responsibilities

• Conduct patient admissions, OASIS assessments, and medical histories.
• Develop and implement individualized plans of care.
• Provide skilled nursing services and coordinate with interdisciplinary teams.
• Supervise LPNs, Home Health Aides, and Personal Care Attendants.
• Submit timely clinical documentation and coordinate referrals for services.
