The role requires an active RN license, clinical nursing experience (long-term care preferred), and familiarity with EHR systems. Strong communication and independent judgment are essential to deliver resident-centered care.

Qualifications

• Current, active RN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one year of clinical nursing experience; long-term care experience, preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Familiarity with electronic health records (EHR) systems.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations.
• Monitor and observe residents’ physical and emotional status; notify the physician and responsible party of any changes.
• Develop and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities and staff education.
