Job summary

Constellation Health Services is hiring a Home Health Registered Nurse to support coverage for Suffolk County and NYC with some Nassau County assistance. The RN will join our Start-Up & Survival Team (S&S) in New York, traveling extensively to patient homes to deliver skilled nursing care and SOC assessments.

You will provide medication administration, care planning, and family education while collaborating with interdisciplinary teams and physicians.

Qualifications

• Active RN license in New York state.
• BSN preferred.
• Strong communication and interpersonal skills.
• Ability to collaborate with interdisciplinary teams and physicians.

Responsibilities

• Deliver compassionate nursing care to patients in home settings.
• Administer medications and treatments as prescribed.

As we expand our footprint, presentation, articulation, and a strong skill set are crucial for introducing our brand. Members of this team, depending on location, are asked to be the backbone of the site.This position is one of a road warrior and requires great organization and task management.
