Job summary

Summa Hospice is currently seeking an experienced RN to provide compassionate, patient-centered end-of-life care in patients’ homes within Akron and surrounding areas. You will lead nursing visits, adjust care plans, and coordinate with physicians and families to ensure high-quality care.

The role requires an active RN license and at least one year of RN experience in acute or home care; hospice experience is preferred.

Qualifications

  • Minimum 1 year RN experience in acute or home care.
  • Hospice experience preferred but not required.
  • Active RN license in state of employment.

Responsibilities

  • Oversees patient care from start to end of life.
  • Conducts initial and ongoing skilled nurse visits per plan.
  • Updates doctor orders in patient chart.
  • Notifies physician of significant changes in condition.
  • Manages interdisciplinary care as applicable.
  • Provides end-of-life care at patient’s home as ordered.
  • Observes and monitors patient conditions.
  • Individualizes and adjusts plan of care as needed.
  • Administers prescribed medications.
  • Promotes continuity of care during admissions/discharges.
  • Counsels patient and family on disease progression.
  • Involves patient and family in care plan development.
  • Maintains clear communication with patients, families, and team.
  • Oversees care provided by CNAs and LPNs.
  • Evaluates treatments and condition per plan of care.
  • Assesses level of care needs and appropriateness.
  • Communicates supervisory visit results with staff.
  • Completes clinical documentation per agency and Medicare guidelines.
  • Documents treatments, assessments, and education.
  • Notifies scheduler of changes outside normal scope.
  • Participates in on-call and case conferences.
  • Attends trainings and mandatory meetings.
  • Other duties as assigned.

Summa Hospice

Schedule: M-F 8A-430P Rotating weekends and holidays

Territory: Akron, Richfield, Copley, Fairlawn, Medina areas

KEY RESPONSIBILITIES
  • Oversees the overall care of the patient from beginning to end of life
  • Does initial, comprehensive, and ongoing skilled nurse visits for patient and family as determined by plan of care
  • Updates all doctor orders in patient chart accordingly
  • Notifies doctor of any significant changes of the patient's condition
  • Manages interdisciplinary care as applicable
  • Provides end of life care in a patient’s home as ordered by the attending physician
  • Observes and monitors patient conditions
  • Individualizes plan of care and adjusts as needs change
  • Administers medication as prescribed by the physician
  • Promotes continuity of care with appropriate admissions, transfers and discharges
  • Counsels' patient and family on the disease/progression
  • Incorporates patient and family in development of plan of care
  • Maintain effective communication with patients, families, physicians and co-workers. Ability to remain calm, have patience and be accommodating. Compassionate and caring while working with patients and families.
  • Oversees and supervises total care of patient provided by nurse aides and LPNs
  • Evaluates the treatments and medical condition of the patient according to the plan of care
  • Determines if the level of care being provided meets the patients' needs
  • Communicates with the LPNs and nurse aides on supervisory visit results
  • Completes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines
  • Documents all aspects of treatment, assessments, and patient education
  • Communicates with scheduler any changes outside the normal
  • Participates in all on-call requirements and case conferences
  • Attends in-service trainings and mandatory agency meetings
  • Other duties as assigned
QUALIFICATIONS
  • One year of experience as a RN in an acute care or home care setting is required.
  • Hospice experience is preferred.
  • Maintains active RN license in state of employment
  • Understands and practices agency policies and procedures
  • Maintains a professional appearance as a representative of the company
  • Ability to remain calm, have patience and be accommodating. Compassionate and caring while working with patients and families.
  • Knowledgeable on nursing best practices. Ability to make appropriate nursing judgments. Ability to identify a situation and handle it with the best possible solution. Detail-oriented and observant.
  • Disciplined style of work ethic. Ability to prioritize and be timely. Works efficiently.
  • Ability to follow directions and work as a team member.
  • Valid driver's license and auto insurance in your name as a driver
