Job summary

Elara Caring is seeking a full-time Registered Nurse to provide home health care throughout Rhode Island, including Westerly, North Kingstown, South Kingstown, Newport, Jamestown and surrounding communities. You will manage your patient visits with independence while receiving support from an experienced clinical team.

You will build trusted relationships, coordinate individualized care, and help patients remain safe and independent in the place they know best: home.

Qualifications

• Current and unrestricted Rhode Island RN license.
• Bachelor’s degree in nursing.
• At least one year of nursing experience in a clinical care setting.
• Home health or hospice experience preferred; transferable clinical experience welcome.
• Ability to travel up to 50% to patient homes.

Responsibilities

• Manage an assigned caseload of home health patients.
• Complete initial nursing evaluations and ongoing reassessments.
• Develop, implement, and update individualized plans of care.
• Assess patients’ physical, psychological, social, environmental, and caregiver needs.
• Provide skilled nursing care in patients’ homes.
• Educate patients and caregivers about medications, treatments, disease management, and safety.
• Monitor patient progress and adjust care based on changing needs.
• Communicate changes in condition to physicians and the care team.
• Coordinate services with nurses, therapists, physicians, caregivers, and community resources.
• Complete accurate and timely clinical documentation.
• Protect patient information and HIPAA compliance.
• Help patients achieve safety and independence at home.

What You’ll Do

What You’ll Bring

• Current and unrestricted Rhode Island RN license

• Bachelor’s degree in nursing

• At least one year of nursing experience in a clinical care setting

• Strong assessment, communication, organization, and patient‑care skills

• Ability to manage an assigned caseload and work independently in the field

• Previous home health or hospice experience is preferred

• Previous Homecare Homebase experience is preferred

• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment

• Must possess a valid driver’s license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.

• Must be able and willing to travel 50%
