Job summary

Interim HealthCare Hospice in Virginia Beach and surrounding areas is seeking a Hospice Registered Nurse Case Manager to provide compassionate, home-based end-of-life care. You’ll join an interdisciplinary team to implement patient plans, monitor symptoms, and educate families about prognosis and hospice goals.

The role offers a strong emphasis on quality of life, flexible assignments, and opportunities for online training and CEUs while maintaining work-life balance.

Qualifications

• Graduate of an accredited nursing program and active RN license in Virginia.
• Minimum of 2 years of nursing experience in Hospice.
• CPR certification and knowledge of home health regulations.

Responsibilities

• Provide home-based care to patients with six months or less to live, focused on quality of life.
• Work as part of an interdisciplinary team to carry out the patient’s plan of care.
• Perform patient assessments, check vitals, evaluate breathing, monitor pain and administer treatments.
• Educate family on prognosis, changes in care plan and hospice goals.
• Take a holistic approach engaging mind, body, spirit and family.

As a Hospice RN Case Manager, here’s a big-picture view of what you’ll do:

• Provide home-based care to patients with six months or less to live, focused on quality of life
• Work as part of an interdisciplinary team comprised of a physician, LPN, CNA, HHA, social worker, chaplain and volunteers to carry out the patient’s plan of care
• Perform patient assessments, check vitals, evaluate breathing, monitor pain, administer prescribed medication and treatmentsEnsure accessibility of medical equipment, prescribed oxygen, medication and other supplies
• Educate family on the patient’s prognosis, changes in care plan, medication and hospice goals
• Take a holistic approach to care that engages the patient’s mind, body, spirit and family

A few must-haves for Hospice Registered Nurses:

• Graduate of an accredited nursing program and active RN license in Virginia
• Minimum of 2 years of nursing experience in Hospice
• CPR certification
• Knowledge of state and federal home health regulations
